Ona, Duchess of Losk, widow of Duke Janush, daughter of Teodoras Kaributaitis (Chwyedorus), donates peasants from the villages of Krivskas (Chrzywszko) and Garadzečna as well as other peasants to the Church of Losk, as well as a tribute of salt, meat, honey and wax from her estate of Losk, and a tithe of grain; and allows to fish in the Biarezina river, to grind grain, to make use of the forest and keep an inn exempt from taxes.

The parchment is torn, decayed along the folds; ink smeared along the folds.
Initial is decorated, taking up 24 lines.
