The parchment darkened in places, is spotty, decayed along the folds, with a cut on the upper right corner.
The first word of the intitulatio is written in capital letters that are decorated. Some letters in the first line are enlarged. On the right margin there is a sign similar to a longer f.

The label and the seal of the Library of the Lithuanian Academy of Sciences are on the reverse of the parchment.
