The diploma by the founders of the Church of Ardilava - Bogdanas Raliuševičius (Bohdan Ralvczevycz), inheritor of Semkova and Ardilava, Ona Anubavičiūtė Rurkienė (Anna Rurkowa Anubowna) with sons and Stanislovas Anubavičius (Stanislaus Anubowicz) with brother Jurgis (Georgius) - to Bishop Albertas Taboras of Vilnius, stating that, following the death of rector (Andrius Andreas), they recommend to approve priest Jacobus from Plock diocese a new rector of the Church of Ardilava.

Only a piece of the black wax seal in a wax bowl attached with a parchment strip remains. Diameter 19x27 mm. The second attached seal is missing, except for the 2 cuts made for the parchment strip.

The parchment decayed in places, with small holes.
Initial is decorated.

The label and the seal of the Library of Lithuanian Academy of Sciences are on the reverse of the parchment.
