Ona, widow of noble Ivaška Davainavičius (Iwaszko Dowoynowicz), donates the following, according to the will of her diseased husband, to the Church of Krupa: her part of the village of Jastrzębla; a pond and a mill in Krupa; a meadow in the village of Olchowo and 2 stones of wax from the village of Sahannyewyce annually.

The document had two seals. Two pieces of the first attached black wax seal and its protective bowl attached with a string of green thread remain. There are three cuts made for the string in the parchment.The second seal is an attached black wax seal of unknown ownership in a bowl of rougher wax attached with a string of cherry thread. There are three cuts made for the string in the parchment. Diameter 22 mm.

The parchment is gnawed by mice in two places along the folds.
Invocatio is written in enlarged letters. Initial decorated, taking up half of the text.
