Ona, widow of Nekrašas Eirudavičius (Nyekrassy Eyrudowicz), and her sons Bagdonas and Žygimantas donate the following to the Church of Bijutiškis and its rector: farms of seven peasants, belonging to the estate of Bijutiškis, and land for the eighth farm; nine corvee - labourers; three plots of land for the rector’s farm called “palivarkas”; two meadows; the forest of Gudai; cattle; tithes from the estates of Bijutiškis, Snov and Černikov (Czerniechow); a "saikas" of honey; a kappa of grosh annually.

The seal is missing. It was attached with a string of cherry thread. Two cuts for the string is made in the parchment.

The parchment is spotty, torn in several places, with a few cuts on the left margin.
Invocatio is written in enlarged letters. Initial is decorated.
