Grand Duke Alexander of Lithuania donates additionally the Ostrovno lake, the Černohost (Czarnohost) river and one peasant from Vitebsk Castle, a plot of unused land Maksymovščina, a tithe from the estates of Vitebsk Castle for the sustenance of the Parish Church of Vitebsk; and allows to keep an inn exempt from taxes.

The parchment is water-damaged, spotty, with a few holes and some rust, especially on the upper right corner. Ink faded.
The first letter of intitulatio is written in enlarged letters. Initial is decorated, taking up 10 lines. There is synchro postscript.

The label and the seal of the Library of Lithuanian Academy of Sciences are on the reverse of the parchment.
