Duke Žygimantas Kęstutaitis of Lithuania donates the tithe from the Geranainiai estate, a plot of arable land, a meadow, a plot of land near the bridge over the Žyžma river, 40 carts of hay annually, and 3 peasants near the Ščara river with their honey and wax duties for the sustenance of the rectors of the Church of Geranainiai, established by Žygimantas Kęstutaitis.

The parchment is spotty, darkened, has small holes. Ink smeared in places. The right bottom corner is cut away. Underwent restoration in 1999.
The first letter of the invocatio is decorated, taking up the whole text. Some letters of the first line are enlarged.

The seal of the Library of Lithuanian Academy of Sciences is on the external part of the plica on the right.
